Children's Ministry Worker Interest Form

Completing this form does not obligate you to serve or to serve at all the times you indicate on the form. The ministry leader will contact you before scheduling anything.

Requirements to Work in the Nursery:
• A member of Grace Baptist Church for at least 6 months (a few exceptions to this are listed in the Child Worker Form)
• Child worker application submitted to the church office and received Pastor’s approval
• Passed criminal background check
• Watched the video about children at risk
• Attended a Child Worker Training Meeting or met with Pastor Stein
